病例描述
一名 56 岁男性突发右眼疼痛伴视力丧失。患者既往无外伤或手术史，左眼视力为 20/20。因前房玻璃体导致房角关闭而转诊。
眼压极高，达 56mmHg，经四类药物治疗后降至 23mmHg。患者期望快速康复以重返工作岗位。
手术目标是通过处理脱位性白内障，实现长期良好效果，避免未来发生晶状体再脱位。
术前评估
术前评估中，术中光学相干断层扫描（OCT）显示前房内存在玻璃体絮状物（A）及严重倾斜的晶状体（B）。一侧睫状沟空间缺失，另一侧虹膜后方异常深陷，玻璃体由此处环绕。晶状体活动度极大，患者仰卧时房角会开放。
